CDI Week

I am really excited about the 1st annual upcoming CDI Week in September. I see on the ACDIS webiste it states "The toolkit will be posted a few weeks in advance of Clinical Documentation Improvement Week." I would just like to suggest for the information to be available at least 2 mths in advance.I know at my facility we will need approval in order to do anything for the recognition. Having it available at least 2 months before will allow for planning and approval. Thanks so much!

    We are having a guest Speaker to address the medical staff regarding documentation. There will be a focus on our Hospitalists and ER Physicians. Also, passing out "candy bags" to physicians on the floor as a little something from the CDI Team and we have arranged to serve a meal in the Physician's Lounge. We have a folder of ICD 10 materials we are going to pass out during that time.

    LOL! Yes as soon as we get it together. We have to run everything through Marketing. Mostly we are focusiing on Root Operations as some of the common definitions of those words are changing somewhat - like resection.

    We are doing a daily documentation tip sheet, planning to take a "happy treat" to a few physicians who have been extremely supportive and for the CDI staff lunch and an educational offering.

    We are not celebrating as "big" as we did last year. Our department
    participates in a "cake fund" to supply a cake each month at our department
    meeting. So, we have ordered some orange and purple cupcakes. We want the
    whole HIM department to join in our celebration.

    We have posters for our office door and the department door. Probably take
    each nursing station some candy bags.

    I wanted to have an open house for the physicians but we don't have the
    funds in our budget for catering. We may make up some treat bags for any
    physicians we see out on the floor.

    Last year we also dressed in CDI colors of purple and orange during the
    week. That was a lot of fun!

    I have some "happys" made up for my co-workers.

    Maybe next year we will have some $$ in our budget for more celebrating.

    Well, its not CDIs budget it's HIMs budget. And as all catering budgets were cut this past year- so was HIMs.

    However, when we were negotiating budget this past year, I requested that CDI have a small budget for CDI Week - for decorations, food, goodies, etc. We were turned down.

    Last year- except for our documentation education guests - all those things came out of our own pockets. And it will this year as well Including the cake fund club which is something our dept. Pays into monthly.

    It sounds like we are all doing the same thing. We really should have a
    small budget I think. I send the physicians' offices that I work with
    closely cookies and take other goodies out. It pays for itself in the
    long run. What do other CDI programs do?

    We don’t have a budget either, but I wanted to do something for my staff – they certainly deserve it. I’m going to order a cake, take them to lunch one day and get them each a small gift. I also liked the Question of the Day that CDI Talk did last year and plan to send out email questions of the day to them with a small prize for the winner. Will probably also bring in bagels or kolaches one morning too.

    Kolaches - Now there’s a pastry you don’t read or see often (especially in the south). My dear Grandmother (whose parents came from Slovakia) made the best Kolaches. My mother makes good ones as well. I like that idea – I will get my Mom to make some this weekend (and teach me how to make them) to offer to the physicians for CDI week.

    West, TX is just a little ways down the interstate from us. It’s a Czech community and have a super bakery. They also have a “West Fest” and “Oktoberfest” every year. Great fun, great food, and great people.

    Hang in there, we all have days like that. I'm glad to hear you have admin support that certainly helps. Do you have a Physician Adviser? I'm trying to get our Medical Director to listen to the webinar from Monday - What's in it for the Physician. I'd love for him to hear it and possibly share it with the medical staff.     Actually we do have a physician advisor, and because this doc has a history of non compliance with the CDS, I did try to call him but he was not working. I did go to the co-director for the Hospitalist program who told me "this doc was in a good mood", so go ahead and take it to him! (well he wasn't in a good mood anymore).
    After this, my CMO has now advised that he is making a policy from now on we are to take that physicians queries to Drs X & Y only and they can take it to him.
    GOOD by me!!!!!

    Hang in there Juli. It happens to all of us at one time or another.
    One valuable tool is if your facility has a code of values which
    includes mutual respect, you can also complain to your chief of staff/VP
    medical affairs about the providers conduct. I've done that in the past
    when I was in case management and it worked wonders and we actually got
    the surgeon to apologize.

    Julie, hang in there. I go into the stairwell to cry as well:)
    MDs need to learn that, in some ways, CDI does represent the government. We are the buffer for them so that the government does not come down on their incomplete documentation with payment denials and low quality scores.

    Bummer, Juli, that you had to deal with someone who is uncooperative and apparently gets some twisted pleasure out of being difficult. I am from the south, we call that kind of person "Trifling".

    It sounds like you did the right thing by seeing your VP and CMO. I usually do not let physicians get me riled up, though many certainly get under my skin - if they start insulting me and the reason I am here - I tell them that I have an appointment to make, and will get back to them. I then usually go see their chair and get a meeting set up or get my physician advisor to intervene.

    I always remind myself and others that physicians are like everybody else - they eat, sleep, die, cry, go to the bathroom, love, blink, grow hair, and have failings like the rest of us. Then I think good thoughts about the situation and pray for them! We are all children of our Creator!

    Have a great fall weekend! Life is great in this wonderful exciting job that we do on a daily basis!

    Exactly Mark. My motto that gets me through those moments is "It's nice to be important, but more important to be nice". It evens the playing field for me. I may not be as "important" as the MDs but I can always be kind and respectful.

    Just a couple take aways on the positive side...
    After those types of interactions, I am always sooo thankful that I am not related to that person
    His tune will change with RAC denials. If his is an outlier, he will be touched by the Medicare Angels. Then all of a sudden he will want you on his team. Been there, done that, and danced when my office blinds were closed.

    I remember the response a UR co-worker of mine used on an especially surly physician one day, "Dr. Jones I don't even let my husband talk to me that way! Now lets sit down and talk about this like human beings."

    I think it reminded him that she was another human being - somebody's wife, mother, etc. Maybe he even thought about his own wife and would he want someone yelling at her that way?! He straightened right up and laughed it off a little and apologized. They were able to work together from that point forward.

    It doesn't always happen that way and I'm, not always that quick on my feet w/responses. Sometimes you do have to go over their heads to let them know you will not be treated without respect and common courtesy.

    I hope all your future encounters w/this physician will be more positive.

    Hi Juli, CDI Week does not offer a raincoat to protect us from the
    "rain". (I'd prefer to use a different word but I won't). Anyway, you
    are doing the responsible and admirable thing for your patients and

    One thing I am seeing missing from the dialogue below is explaining to physicians what is in it for him/her or the old WIIFM. Talk about quality and safety initiatives generally do not persuade physicians to improve their clinical documentation, these concepts are just too intangible for physicians to really be interested in, given their struggle to earn a living in today's current economic climate of decreasing reimbursement. One may try to incorporate the concept of clinical documentation as relates to the establishment of medical necessity for the physician's own business of medicine. Physicians today are experiencing many more denials and downcodes for patient services by Medicare and other third party payers attributable to lack of medical necessity for their billing of a specific E & M. Medical necessity consists of the intensity and frequency of services provided, for an inpatient stay, medical necessity is measured by the level of E & M billed and how many days the patient was in the house and the physician billed for each day. You could learn the five steps to correctly billing third party payers for physician services and integrate CDI education to physicians. You will likely meet with much more success in communicating the necessity of complete, accurate and succinct clinical documentation from a CDI perspective. To this end, you won't be wasting the physician's time and even as important, your own time.
