physician certification

Has anyone developed or seen a physician certification form for medicare inpatients? Also, is this required or just recommended by CMC for the Final Rule affective 10/01/2013?

    Ours are incorporated into CPOE admission orders and then a form was created for those physicians not on CPOE yet.

    This is what we are utilizing. We do not have CPOE yet.
