I went to the ACDIS National conference last year. I did see the 'Obstetrics: A new beginning for Clinical Documentation'. Trisha McCauliff Streb RN BSN CCDS was the presenter. Like many facilities we are dealing with 'maternal drug dependence' and it is increasing statistically. I believe we have opportunities here. What is the code for maternal drug dependence? Would anyone have the exact financial reimbursement numbers for a mother who had a vaginal delivery with this secondary diagnosis and a C-section with the same 'maternal drug dependence' as a secondary diagnosis? It is a lot of time and effort on the part of Case Management and Nursing to care for these patients and their newborns. Very situational.

    648.31 is the dx for drug dependent moms with an additional dx for the type of drug, e.g. 304.01 for opiate dependence, continuous use. This dx is unfortunately NOT a complication or comorbidity for the mother who delivers vaginally and the DRG remains 775 - vag delivery without complicating diagnoses, with a very low weight of .5643 for NSVD. For a C section, interestingly, 648.31 IS a CC/MCC and moves the DRG from 766 to 765 and bumps the weight from .7562 to 1.0924.

    For full term babes born with 779.5 - newborn drug withdrawal syndrome and no other problems -the DRG bumps up to 793 with wt. 3.5968 "full term neonate with major problems." This is a huge jump from DRG 795, normal newborn, with a weight of .1724. Ask one of your coders to run these codes through your system to get exact reimbursement numbers for you

    We also are seeing more moms who are drug dependent but find that caring for the newborns is much more intensive than caring for the moms. The baby's LOS is sometimes a month or more as they are weaned.
