Anesthesia record for ICD-10

I had recently seen a sample checklist/anesthesia pre-op form with some ICD-10 concepts incorporated in it, but for the life of me, I cannot find it again! Our anesthesia department is getting ready to update their preop checklist and have enlisted our assistance. Does anyone have a sample they would be willing to share so as not to "re-invent" this wheel? Or perhaps you can direct me to a website that has some samples. I could not find anything in ACDIS. Thanks in advance!
