Malignant pleural effusion

Hi all,
I just had a discussion with a coder where she referenced a recent case where billing came back to her for re-sequencing of a malignant pleural effusion because they said Medicare would not accept it as Pdx. Upon further review, under J91.0 there is a 'first code underlying neoplasm note'. This seems to go against our usual practice (per guidelines) of coding the complication of the neoplasm as Pdx with the neoplasm as Sdx. Anyone else seen this issue? Are you coding the pleural effusion or neoplasm as Pdx?

    Malignant pleural effusion J91.0 is a manifestation code and, as such, cannot be sequenced as principal diagnosis. See coding Guideline I.A.13 Etiology/Manifestation conventions.
