Question on CC for CAD w/Bypass Graft

I need clarification on when to use the CC of "CAD w/bypass graft". This has come under question by our coding team. I am trying to find a coding clinic on this. Any help would be appreciated.

    A patient who has had a CABG in the past and who develops new symptoms (unstable angina, if new onset angina) may have the new symptoms caused by a new narrowing of a native vessel or narrowing of a bypass vessel (whether LIMA or vein graft) and the only way the doc can say which is with a coronary cath demonstrating this narrowing. Now, just because there is narrowing does not mean that there is a link. Many 65% lesions of a larger vessel can be asymptomatic whereas a 30% narrowing of a small vessel can cause extreme symptoms. The doc must link the narrowing to the symptoms.
