Coding CKD stage 3-4

If a provider documents "CKD stage 3-4," do you code to the highest level? There is some debate between coding and CDI about coding this scenario. Thanks!

    that's what I have been doing is coding to the highest level, unless of course there is an issue with validation based on lab studies, etc

    Could this be considered conflicting documentation which would require clarification?

    At previous employers we always coded to the highest level when it came to CKD, unless the scenario is as proddy@clarionhospital.org described. I don't think that's conflicting. Thanks for the input! If anyone has anything to add, please do so.
