Using the term "provoked" in reference to "Post procedural pulmonary embolus"

A provider has documented "Provoked Post procedural Pulmonary Embolus" as the primary discharge diagnosis.  Patient had a recent successful cardiac ablation 6 days prior.

Should I query if provoked means that it is a complication or not?

    We submitted this exact question for coding advice to our vendor.  They answered that it needed to be queried  because the documentation did not clearly link the condition to the surgery.
