Acute on ckd3

If a doctor writes acute on ckd 3 are we able to code acute renal "failure" and ckd 3?

    No, you cannot not code acute renal failure from "acute on ckd 3".  They must specifically document acute renal failure.  I would however code the CKD 3 from that statement.
