Cardiomyopathy with Htn , CHF and CKD

This 47 y/o patient has non-ischemic cardiomyopathy, hypertension, chf and ckd. If the doctor suspects the cardiomyopathy and not the hypertension is responsible for the chf, then the hypertensive heart w ckd and chf (I13.0) should not not coded, correct? Or can they both be coded?

If the provider stated that the CHF is not related to the HTN, then they should not be linked (using the combo code).
