The ICD-10-CM and ICD-10-PCS Coding Handbook gives a good explanation. In short,
Excludes 1: the code excluded should never be coded at the same time as the code above the "excludes1" note (unless codes are unrelated); you assign only the code referenced in the excludes1 note with two related conditions.
Excludes 2: the condition excluded is not part of the condition represented by the code. You are allowed to code both the code and the excluded code together if a patient has both.
