Midrange CHF

Good Afternoon! We are starting to have documentation of midrange EF in addition to HFpEF and HFrEF with our CHF patients. Anyone with an EF of 40-50% has the CHF documented as HFmrEF. How are you capturing the "midrange" or are you initiating a query? I am not sure how to compliantly query to get a diagnosis that is able to be coded. I was looking for some input before meeting with cardiology.

  • Thank you. We are actually having them document HFmrEF so we are in a bit of a conundrum.

  • There is not a specific code for midrange. You should query for diastolic, systolic or combined forms. The term midrange has no reference in ICD-10 and should not be coded to diastolic, systolic or combined.
