How to code CKD 3-4?
My colleagues and I are trying to determine if there is a set policy or guideline for coding of diagnoses that providers use a range for.
Such as:
CKD stage 3-4
Protein calorie malnutrition, moderate - severe.
Do you code to the highest level documented, or query for clarification?
