Queries as part of the medical record

Hi All,

I am looking for any information that supports making a CDI query part of the medical record that can be coded from, is best practice. Most of the references I have found so far say it is up to the individual facility/ health system to determine whether or not they want queries to be part of the legal medical record. What do your facilities/systems do and how did you arrive at that decision?

    In my experience I have also found that all the resources (Updated 2022 Guidance for a Compliant Query Practice) say it is up to the individual facility to make that decision.
