Coding to CDI query requests and communication

I hope you can lend me some help with the topic of Coding/CDI query requests and communication.  I am working with my coding manager to solidify a process that works for all and so far have some roadblocks that I am hoping can be worked out with finding out what others in the industry are doing.


What is the process or policy for query requests from Coding to CDI?

Are there specific query types (DRG shift impact, complications, etc...) that coding always sends to CDI?

Are there specific query types that coding never sends to CDI?

Is there a conflict resolution in place for when CDI does not agree with the request?


I appreciate any help you can provide.

    If Coding finds CDI needs to query, they send it to me, I will review the record and determine whether a query should be sent. I will provide documentation from the record and rationale as to why I sent or did not send the query. If we still disagree, we will have the Coding Quality Supervisor review the chart.


    Most often we receive requests for Obesity, Malnutrition or excisional Debridement or clarification of a diagnoses. These do not come back often.

    The only query we do not get back is for Pathology. Our IP or Quality Coding Supervisors will submit those.

    Any conflicts are resolved with the Coding Supervisors and if needed the Coding Manager.
