Pressure Wound vs Pressure Ulcer
I have seen lately a number of physicians documenting full thickness pressure wound to xxx, I always thought the words ulcer or injury needed to be present. I am not able to find anything new out there so I am asking for confirmation or information on any new changes.

I would also like clarification on this... The Alphabetic index does not allow for a pressure wound to be classified, and a Coding Clinic search was futile... so I think a query is required for complete clarity. Anyone else find otherwise?
We query when "wound" is documented and educate providers that "wound" equals "injury" which is a S code when if what they really mean is "ulcer" (L code) they should state "ulcer" and include the deepest layer affected if non-pressure as well as etiology and POA status.
