If we have patient initially admitted for sepsis and intubated hours, later diagnosed with perforation undergoes surgery - how would coders code principle diagnosis and either use DRG 870 or procedural code.

It sounds like sepsis would likely be your Pdx. If they have a reimbursable procedure, that will drive your DRG. Ideally your procedure should be in the same MDC as your Pdx (18). Depending on the details of the procedure you're likely DRG will be 853 Infectious and Parasitic Diseases with OR Procedure with MCC (presuming it is a bowl perforation giving you an MCC). 870 would be the DRG without reimbursable procedure.
