Cardiothoracic Surgery

Anyone have any great resources for Cardiothoracic Surgery? We have a resistant surgeon we are putting some resources together for. Mainly focused on CC/MCC capture and increased CMI. I have put together CC/MCC lists and info about coding rules and impact secondary dx. Just wondering if there is something I am missing. I’m in a time crunch with 3 working days until maternity leave so I figured its worth seeing if anyone else has anything great.

I appreciate any/all help!

  • CMI is not going to impact him personally. I explained to my CV surgeons based on SOI, ROM and LOS. They really appreciated the "extra hospital days".

  • Our physician advisor is a surgeon. She is a wonderful resource for one
    on one discussions with challenging Docs. Sometimes they get lost in our
    minutiae if you don't give them anything in their own vernacular. CMI
    means a lot to hospital administrators but your surgeon may listen if
    you address his/her "what's in it for me" point of view. Does it help
    them justify a higher modifier level? Does it positively impact their
    health grades by reflecting a higher level of acuity and justify
    resources used? I.e. your patient's are sicker and you need to take
    credit for the care rendered. Good luck.

  • Yes, I agree. He will be presented with all the ROM/SOI/LOS data as he has in the past. This is be driven by the quality team and they seem to be focused on the fact that his CMI is low (whereas hospital CMI is high). They are hoping to show him in yet another way that he is not adequately reflecting patient severity.
    The materials I am presenting include info on LOS and SOI/ROM (CC/MCC capture), I have just added a piece about how this can ultimately impact CMI.

    Have you tried showing him his CMI/LOS compared to an associate's? If only 1 CV surgeon, are there general surgeons who also perform some of the same surgeries (for example: we have a gen surgeon that also does fem-pops, fem-fems, etc) that you could compare him with? They are usually pretty competitive and this might encourage him to improve his documentation. I have 2 that will work fairly well with me and one that told me "I don't give a *%#@!". So unfortunately you can't win them all.

  • He's our only. We are comparing him to CC/MCC capture benchmarking data for his DRG's that we have from our consulting group. We also have quality data from that we can compare him to cohort on. We have the data but in the past he has also been in the "I don't give a *%#@!" category. My soon-to-be manager (we are transitioning to quality soon) thinks he has foraged a new relationship with him and wants to give it a try.

  • I have several different things. I will not actually be there for this discussion as I will be on maternity leave. Its awkward because the person who will be talking to the physician is not a CDS. He will be our manager when we transition to quality. He is great, but still learning. So, I have provided him with a billion resources both for him to help support the conversation as well as tools for the Surgeon. Most are probably things I Have posted on here, but I will include a sample.
    He will also have our consultants data comparing his capture rates to cohort. Plus Crimson data and Compass data.

I have put together CC/MCC lists and info about coding rules and impact secondary dx. Just wondering if there is something I am missing. I’m in a time crunch with 3 working days until maternity leave so I figured its worth seeing if anyone else has anything great.

  • Yup, we have all that for him from multiple sources. Crimson provides all his quality data. SOI/ROM, CMI, CC/MCC capture, LOS data, O/E ratio, complication rates, etc. Compass compares his MCC/CC capture as well as shows ICD-10 weaknesses. This is all can be compared to peers/cohort/state/national/etc. Then we have our data from our consulting group regarding his CC/MCC capture data broken down by specific DRG and compared to benchmark (80%).

    I have several different things. I will not actually be there for this discussion as I will be on maternity leave. Its awkward because the person who will be talking to the physician is not a CDS. He will be our manager when we transition to quality. He is great, but still learning. So, I have provided him with a billion resources both for him to help support the conversation as well as tools for the Surgeon. Most are probably things I Have posted on here, but I will include a sample.
    He will also have our consultants data comparing his capture rates to cohort. Plus Crimson data and Compass data.

I’m in a time crunch with 3 working days until maternity leave so I figured its worth seeing if anyone else has anything great.
