SOI/ROM for pressure ulcers

This question is for those of you that have access to SOI or ROM software. Does the documentation of stage 1 or 2 pressure ulcers change the numbers on the SOI or ROM? I am trying to work with our wound care nurse to get a better documentation of pressure ulcers in the concurrent record from her so I can refer the physicians to it and do not have any way to show her that the SOI or ROM would change it we could get these documented then by the physicians. I would assume the numbers would change with documentation of pressure ulcers but can anyone verify this for me?

    All stage of pressure ulcers even unspecified stage are SOI & ROM drivers

    We receive a wound care report each morning. This is created from a collection of consults ordered by either the physician and or the floor nurses. We review every chart on the list. This is a collaborative effort as pressure ulcers are reported via leapfrog and healthgrades.
    Our goal is to identify them immediately when they present, treat them and monitor so as not to have a increase in stage. It has taken us a while to build this collaboration with wound care so do not give up! Good Luck.
