I feel like I am always leaving queries to specify the acuity of diseases. I am leaving queries that have true clinical indicators and enough information to justify the query. Our physicians are notorious for writing diagnoses such as COPD, malnutrition, CKD, anemia without further description.
Do any of you have this same issue? Is there a limit to the queries?
