At our institution we code Pneumonia as principle with Pleural Effusion
as CC. Based on previous experience we use the coding clinic guideline
for CHF/ Pleural Effusion that states if the M.D. does not state that
the Pleural Effusion is unrelated to the CHF ( or Pneumonia in this
case) that the Pleural Effusion is considered part of the Pneumonia
disease process and will be coded secondary. Pleural Effusion can be
coded as a CC because it was treated by performing thoracentesis.

I have a scenario I need help with. Pt came in with pleural effuson,
unknown etiology. No hx of CHF. Thoracentesis was done. Fluid was
clear and straw colored. No WBC's. Final dx was "parapneumonic pleural
effusion" and "community acquired pneumonia".
I am thinking it would go to pleural effusion but coder is taking it to
pneumonia. No coding clinic reference has been found yet.
Any feedback would be helpful.
