CDI and Core Measures

Hi Everyone,

For those CDI programs that do not track Core Measures concurrently during the CDS review, I would like inquire about how your CDI program helps make an impact on Core Measures Compliance. How can CDI help maintain or increase compliance with Core Measures?

We are looking for creative, proven ways to ASSIST with Core Measures like:

Skimming charts during the CDI review, review the 4 major measures when assigning Pdx specific to those areas, work with Core Measures to do MD education?

    I added a "Process of Care Core Measure Reminder" to my queries for PNA, Sepsis, MI, CHF, etc. I include the core measures bulleted out. These are the queries most often placed! (other than tx w/out diagnosis)

    The reminder on the query is more to prompt them (the physician) that this is a core measure diagnosis and to please make sure they address the indicators. The follow-through/auditing of the chart for the presence of that indicator is not my purview, but if I notice something missing, I will address it with the physician.

    Attached is the PNA query I created (with the gracious help of many here) and it has the process of care core measure reminder.

  • I print a daily report listing all the patients I have reviewed and thier core measure diagnoses and give it to both Quality and Case Management. The report also includes GLOS and actual LOS. I use the 3M CDIS software and it is an already available report.
