Acute/Chronic Cor Pulmonale

Would anyone be willing to share their "Acute/Chronic" Cor Pulmonale Query - if you have one?

Acute Cor Pulmonale is an MCC and even so - we get quite a lot of COPD'ers' in our patient population who meet clinical indicators for this diagnosis - Ac or Chr.

It would be greatly appreciated!!

    Yes, it is common in COPD, ILD, some neuromuscular disorders, sleep
    apnea (pulm. HTN) and other diseases where a patient is in chronic
    hypoxia.

    Can I offer a suggestion? Have your pulmonologist address your task force
    on what he/she thinks the definition of cor pulmonale is, then combine the
    criteria and their own words in to a facility specific query. I am very
    cautious to use the "criteria" as the basis of queries as it is becoming
    more common for payors to deny based on their own reasoning/criteria. If
    your query is formatted and supported by your own medical staff, they will
    be more likely to defend it in the event of a denial. I recently reviewed
    to separate request for medical records with acute renal failure, where
    payor A was denying and using the RIFLE criteria, and payor B used the
    AKIN criteria.

    That's an excellent suggestion. I believe "facility/policy" approved
    queries are going to be used more and more as we begin to see the
    results of many audits and reviews.
