Endogenous Depression

We recently underwent a CDI chart audit. One of the consultant's recomendations is to query for endogenous depression if "depression" only is documented.
The code for endogenous depression is 296.20, Major depressive disorder, single episode, unspecified, for major depression.
This code is a cc.
Two fold question:
1)Are you seeing endogenous depression documented by physicians?
2) If not, and depression is documented are you querying for further specificity of depression? And if so, would you be willing to share query sample?

    I don't normally query for depression because while many of our patients have it, not all are seen by a psychiatrist or mental health NP so we could capture the code and hospitalists remind me that they are internists and not mental health professionals. We do have a query form for this, but have never heard of the term endogenous depression before much less seen it documented by any of our numerous mental health providers. I do frequently see Major Depressive Disorder documented.

    Most of the physicians I work with refer their pts to a psychiatrist so they won't document that specifically since they are not the treating physician.

    I think I see a query opportunity here. When entering into the encoder:

    depression, mental, endogenous, single episode/unspecified

    then your choices are: mild, mod, severe {with/without psychotic behavior}, remission {partial/unspecified/full} and unspecified

    A query would obtain the highest level of specificity.
    All give a CC except the remission codes give a CC.

    Charlene,

    Our issue is that "depression" only is documented in the chart, endogenous depression is not documented in the chart nor treated with anything but regular home meds, so not sure our hospitalist are going to be comfortable with documenting endogenous depression. We are going to meet with our physician champion next week.

    If I only had depression, I would not query either.

    But if someone does document 'endogenous depression' for further specificity one could query for mild/mod/severe/remission status.
