data collection and worksheets

I am wondering if anyone who works without a consultant company would be willing to share how they do worksheets and an example of one....where they collect data, what data collected and examples of a spreadsheet or however it can be done. I would greatly appreciate any help. My hospital recently has decided to not use a consultant and/or software so I am doing everything by hand now.
