Clarification of TACO and TRALI
To clarify the two conditions-- TACO or transfusion associated circulatory overload presents with dyspnea and elevation of the blood pressure during the transfusion. By stopping the transfusion and/or giving IV diuretics, this condition is usually self limited. TRALI or transfusion related acute lung injury presents with fever, dyspnea, hypotension, pulmonary edema and ARDS, usually secondary to transfusion of plasma and platelets. Rarely seen with RBC transfusions. This can occur anywhere from 1-6 hours post transfusion and is the leading cause of transfusion related fatality (mortality of 40-50%)These patients require vent support. With TRALI, the donor has an antibody against the recipient's HLA (Human leukocyte antigen) This is obviously a much more serious condition than TACO.
