cardiac arrest

I need some help from my Coding Wizards! Patient came in - cardiac arrest due to sustained V-tach, respiratory failure.

I am familiar with Coding Clinic, Second Quarter 1988 "Cardiac Arrest, 427.5 and understand the sequencing of the V-tach followed by the 427.5 But is there a coding rule that the respiratory failure can't be used as the PDx since it was also POA??
