Query on level of Mental retardation

I am looking at querying regarding the level of MR on a surgical patient and would like to know if anyone has any resources they have used or a sample query they have used in past. My research shows only that the IQ is the most commonly used indicator to determine the level of MR. Does anyone else have another reference they have used successfully in querying as I don't have an IQ on the patient?

    Specificity of Mental Retardation is a difficult diagnosis to document
    unless the physician or provider knows the patient's history. For a
    Surgeon I would imagine it would be more difficult as they are usually
    providing a service (surgery) and nothing more. You may obtain specificity
    from the Medical Consult - if you have one - but most will not know the IQ
    scores of their patients. They may obtain if from family members - if the
    family knows this information. You could venture a query listing the
    appropriate diagnoses and query choices. Perhaps you will get lucky and one
    of the providers or family will know this information.
