We are putting the final touches on a revised "The Physician Documentation Improvement Pocket Guide" by Pamela P. Bensen, MD, MS, FACEP, the laminated card that comes in packs of 25. I am looking to see if anyone has the original version and if they would be willing to let us know why/how you used it.
