Existing Query forms

Does anyone have a generic query form and/or for the following diagnoses/diseases they would like to share?

COPD
Diabetes
Hypertension

    I use the HTN and Diabetes query forms from the ACDIS forms and library.
    Would like to have on for COPD.

    I tried looking for HDN and Diabetes in the library but came up empty. I will look again.

    Brother! I already have the Hypertension - forgot to check it off my list! long couple of weeks. :-)
