Physician Education

Hello all! Our department was wondering about how other facilities conduct their physician education process or if you even do it at all? We are required to do one physician speciality group a month on a specific topic. Is anyone else doing this? If so, what is your process?

    Yes, I try to get time at the staff meetings and I will usually pick the topic based on what issue generated the most queries recently. I also let them know how this information will benefit all of us (physicians, CDI and coder's) in ICD-10.

  • We put out mailers 1-2x month- "DOC-u-tips". That explain specific documentation needs. This allows us to solve a problem sometimes while it is happening (quickly produce due to trend or identified need coming fromt he coding staff). These are also posted in their lounge. Past doc-u-tips are also available through a link on all computers within their work areas so that they can revisit in a time of need. They are quick reads- 1 page- large print.

    We also write an article in their monthly newsletter on a hot topic.

    I also target specific physicians with items in their mail.

    We have also placed reminders- updates, specific criteria such as CKD stages, SIRS criteria, acute respiratory failure criteria etc in charts. We have a clear sleeve in front of progress notes that is our "ad space". this is not placed due to a specific documentation need but placed in all the records.

    The best and most successful method is face to face- one on one....

    I have a bi-weekly newsletter I send out via email to all providers on a documentation related issue. In addition I orient all new providers one-on-one and will also meet with providers individually if additional education is needed.

    Hello,
    We have a standing invitation to visit with our hospitalists once a month for about 10 minutes. We are a permanent part of the agenda.
    I am very careful to only present one or two ideas and not to go over my allotted time.

    Their attention span is very limited.
    Sometimes, I bring handouts related to a topic, ie: Sepsis, Functional Quadriplegia, Nutrition etc.
    At other meetings, I will bring their individual score cards. They enjoy seeing how many queries they have answered and their financial
    Impact.

    At the very beginning of our program, we went to every service line meeting and introduced ourselves.
    I do have to say though, the best way to work with your physicians or provide education is one on one, on the units.
    I believe when they can relate it to a specific patient, sometimes, it has a higher value.

  • Yes- 1:1 with new physicians is huge. They then know who you are and why you are there. This sets the tone for future interactions and compliance with expected query rates.

    Formally I attend physician staff meetings to present documentation tips and present 1 CME a year to the physicians.

    I always start with a record audit (generally 10 records per physician) to see what documentation we are missing, what they do well, is documentation done in a timely manner, I also usually go ahead and check that orders are cosigned/timed/dated (for QM). Then meet with each physician individually either in their office or here at the hospital to review the results. I've only been able to do this with a few groups and wish I could get buy in to do this for each specialty/individual physician. Most physicians were receptive, although a couple flat out said they didn't care and weren’t going to change the way they document. A few gave me the "just let me know what you need and I'll be happy to help..." and then ignore the verbal and written queries any way. But there were quite a few who actually changed and improved their overall documentation. I have one physician whom I have never met personally, but who called me a couple of times to ask about queries that I had left. Since then, if he has a question he will either call me or he'll leave me a note on the query asking for more information, parameters, or definitions which I then fax or mail to his office or will leave on the record in the progress notes for him to pick up and take with him.
    We have purchased the pocket cards that Dr Benson sells through HCPro and distributed those also. I wish we had a physician advisor, I think it would be much easier to get in to see the physicians. I talk with them on the units when I see them, but they are usually in a hurry and it goes in one ear and out the other

    I took it a step further and worked with our Physician Director of our
    residency program. We required all residents to round through the CDI
    department prior to caring for inpatients. It was a 4 hour block of time.
    They learned basic DRG/Coding and documentation principles (101 Course)
    and then had to "Be a CDS". I had them go out at get response to
    unanswered open queries from their fellow residents on the floors. It was
    a great learning experience for the residents! It built great
    relationships between the CDI team and the resident and well as educated
    them on the importance of documentation.

    Thanks everyone for all your responses! How do you feel about providing education in a physician's office? We sort of feel like it's their personal space and maybe it should just be done in meetings or with newsletters, but then again, it's one-on-one and could be effective.

    We do a "DIM REPORT" every quarter that CDS, Coding, Health Information Services etc. put articles in related to what we do. So the CDS, we usually put one article in their about new information, queries, core measures are areas we focus on. We also have a yearly meeting-which is coming up in December-(so any ideas for that would be appreciated)where we do an educational presentation on something-CDS. Usually do a powerpoint for that if possible. Would be glad to share some of our topics if you like.

    There are several options from consulting firms that have been mentioned (Pinson, Haik, Gold, Kennedy, Brundige). I'd add also Dr Garry Huff who we've worked with.

    You might want to take a look at the conference materials for sponsors and exhibit hall attendees.

    This also relates to what I've been working on for a year now -- building an internal team of Physician Advisors (obviously great clinicians but also expert in CDI/coding issues and working with Medical staff, quality, IT, etc.).

  • Just a thought-- there is a very short presentation, I think originated by 3m on acdis site under forms and tools, it's about APR -DRG but it contains the basic premise of "capturing" all secondary diagnoses. If you are interested and need help locating let me know and I can (tomorrow).
