Too leading?

Hi, needing some opinions from the group on whether or not it is considered leading to ask a physician to clarify if the patient's gastroparesis is due to the diabetes and give the options of yes, no or clinically unable to determine for the query answers?

    How about wording such as....

    A cause and effect relationship cannot be assumed per CMS guidelines unless documented by the physician

    Please document the relationship if any between the gastroparesis and diabetes

    maybe:
    Please clarify if Gastroparesis
    due to diabetes
    unrelated to diabetes
    other
    unable to determine?


    OR

    Please clarify if there is any cause and effect relationship between gastroparesis and diabetes?
