Docu-bloopers

At our recent conference I heard several attendees mention that they "collect" documentation bloopers and either post them in their office, collate them into a folder or otherwise save them for a chuckle.

I would love to see a section on the ACDIS website (possibly with cartoons) as a regular feature. Does anyone have de-identified "bloopers" that you'd be willing to share?

Here's one of my personal favorites:

Patient admitted with difficulty breathing and inconclusive CXR. H/P dictation states "will get paleontology consult" to evaluate respiratory status". I knew the patient was old, but really!

    My all time favorite is intraocular breast implants. Gives a whole new meaning to "my what big eyes you have!"

    I have seen in the progress note of a patient with dehydration "will monitor her eyes & nose" and also for a O2 dependent copd patient "this patient has homo two" - clearly a transcription problem, buy funny.

  • "old farmer's disease" instead of alzheimers. Seems very appropriate for Iowa!
