readmission programs

I have a question regarding readmission programs currently in use at different facilities. We are researching some of the more visible programs and would love to hear what is working out there for other facilities. We are reviewing the Project Boost, Staar, and the Project Red programs in particular.

Any comments or suggestions on which way to head in our search?
