SOI/ROM

We are medium size community hospital. I've been directing my CDI staff to not leave a mortality review that comes out to less than a 3/3. However, recently a consultant told me they should all be a 4/4. In the setting of this particular hospital and the patients we see that is not always possible.

Any thoughts or suggestion would be very helpful.
