RE: Malnutrition Policy

Our process is similar in that when I see the dietician document malnutrition during their assessment, and most of our patients are seen by dieticians, I query the provider for the clinical significance of the dieticians note and ask for a level of severity. Our RD's are very good about documenting the presence, type, and severity of the malnutrition as well as nutritional risk and nutritional injury score. I also don't have a policy to work with nutrition, but we do get along well and communicate well with each other when questions arise.
