We have a template in our EMR that they check mark the appropriate item when they review the case. The IQ review itself is not part of the medical record but it is sent with the record at the time of an audit. The template items include that the chart was reviewed and 1)met IQ Inpt criteria or 2) Did not meet and was sent to EHR for second level.
