Polycystic Kidney Disease and Dialysis

Good morning! Happy 4th of July. I came across a patient who was admitted for symptomatic hyponatremia and is on CAPD due to polycystic kidney disease. The doctor has not mentioned ESRD or CKD stage 5 or anything else except the PKD with CAPD. The question that I have is, do I need to clarify with the doctor that the PKD has caused renal failure, so that is in writing, or is that assumed since the patient is on dialysis? Tried to find information on this and couldn't find any. Thank you for any help or insight you can give.

  • There should be a lot of clinical information on the web.
    Adult PCKD consistently leads to failure, ESRD, dialysis, etc.

    Looking a bit through the encoder we use, I don't think you can get anywhere without the physician documenting either ESRD or CKD 5 (where the patient requires chronic dialysis) to be able to code 585.6 -- even though that is the situation.

    Though I am not a coder -- I believe your current coding is
    PKD -- 753.12 Polycystic kidney, unspecified type
    or
    Adult type PKD -- 753.13 Polycystic kidney, autosomal dominant
    and
    CAPD -- V45.11 -- Postprocedureal renal dialysis status

    I would ask for ESRD due to PCKD. Will you see the Nephro? This would be a good verbal query.
