excisional debridement
I kinda feel like an idiot for not identifying this issue earlier but here it goes….
It recently was brought to my attention that we can code excisional debridements done by PT (true?). I have a sneaky suspicion that this is not happening because the coders are not looking at PT notes and the MD’s are not mentioning the debridements in theirs. I am wondering what processes other hospitals might have in place to streamline to documentation process for accurate capture. I am assuming the MD has to mention the debridement and then they can code off the PT note? How do you address incomplete documentation from the PT?
Anything else? What am I missing?
