IPPS 2014 - 2 midnight ruling

How are CDI programs assisting with the new IPPS 2014 ruling requiring documentation from the physician to write an order for inpatient status and the reason for admission for at least a 2 midnight stay for the patient. We use a case management protocol at our facility.

    Here at the Alamance Campus of Cone Health, CDI actually facilitated the discussion and pulled the work group together! Our Intake Department and CDI met with the Physician Champions and our IT reps to form a new order set that incorporates the necessary parts of the certification. Given the Physician Champion involvement, the new policy was readily accepted and well perceived! We completed the educational flyers/posters that were then presented to the Medical Executive Committee for hospital wide education. The 2 Mid Nite order set will replace the pre-existing admit order sets which are no longer available for use. The EMR will no longer allow an inpatient admit order to be placed without the appropriate documentation certifying the need for 2 midnights, inpatient only procedure, or the "rare and unusual circumstance" with adequate documentation.

    Anyone can download a copy of the sample letter. It just so happens that the "Report on Medicare Compliance" sample edition is current. Yes, it does cost to join but the sample is free. We still have our certification letter in draft form but are utilizing their recommendations. I don’t have one to share at this time but page 3 of the report helps a lot!

    Has it been discussed here that Congress has asked CMS to postpone the rule, and CMS announced 9/26/2013 that it will "delay scrutiny of short inpatient stays for 90 days to allow providers to prepare for the new policy."
