Physician thank you letter

Hi everyone,
As part of CDI Week, we encourage folks to send a letter to their physicians thanking them for their documentation improvement efforts.
We have a couple of "good documentation" certificates in the F&T Library but I was wondering if anyone in the Talk group would be willing to share a sample letter or two that we could post to the CDI Week page to help people out?

    I have bought generic thank you cards and wrote personal notes in each pertaining to that physician and included a few sm chocolates with each.

    I have given out a "certificate of excellence" to residents &physicians
    (see attachment for template) and included chocolate and/or candy with
    it.
