You are absolutely correct. You cannot present a new diagnosis or even a diagnosis from a previous encounter. You can only present the clinical indicators and potential diagnoses (more than one) along with other and unknown to ensure you have a compliant, non-leading query. You may want to ask the JA Thomas folks if they have read AHIMA "Guidelines for Achieving a Compliant Query Practice.", Journal of AHIMA 84, no. 2 (February 2013): 50-53.
