We do not have institutional definitions and as I am thinking more about this type of query, I am realizing how helpful these definitions would be for situations like this.
My fear is that this provider is so used to being queried that they are documenting this dx inappropriately to avoid a query or because they believe that it is appropriate whenever there is a dip in H&H.
