I do what you do Katy. We have all Hospitalists and if one did a note
and then is off for a week while the next covers, the one on duty gets
the query. I do make it clear that it's not their documentation being
questioned and I do follow up with the author of the note queried for
educational purposes so they know what is going on. Of course my
preference is always to go to the notes author whenever possible. The
challenge is when it isn't possible and timeliness is a big issue for us
since charts have to be coded within 7 days of discharge.
