sp cath
Can anyone tell me if they generally query for UTI due to SP cath. We normally do for indwelling, but do you for an SP cath?

The 3M Nosology team recommends the assignment of diagnosis code 596.81 for an infected suprapubic catheter in a cystostomy; assign 599.0 for the urinary tract infection and an additional code to identify the organism, if known.
The suprapubic catheter is not an indwelling or urethral catheter, therefore code 996.64 would not be appropriate. The new code (effective 10/1/11) for infection of cystostomy was discussed in the AHA Coding Clinic® for ICD-9-CM, Fourth Quarter 2011, pages 128-129:
· when a person has problems with urination, a suprapubic cystostomy may be performed to create a connection between the bladder and the outside of the body. A catheter is placed through the abdominal wall above the pubic bone and into the opening to drain urine from the bladder. This procedure is usually performed when a catheter cannot be advanced into the bladder through the urethra. Urine is drained from the bladder and into a collection bag. The catheter is kept in place until scar tissue forms a tract between the bladder and exterior abdomen. Once the tract forms, the catheter can be removed periodically to prevent infection. When the cystostomy is no longer necessary, the catheter is removed permanently to allow closure of the tract. In addition to infection, complications of cystostomy include leakage of urine, peristomal breakdown of skin and failure of the tract to close.
