What happens to me,I see the opportunity say the BMI and not addressed or when I add all the pieces I see malnutrition as a possibility and no dietary intervention or the pt hasn't been weight. I tap the nursing staff for a possible dietary consult . Bases on the info I see in the chart.
We then have evaluation, monitoring and treatment . Everyone wins, starting with the pt, no chart deficiency for the nurse or aide , no chance of having to do and appeal. And follow up by medicine and dietary.
