ICD-9-CM code for AMI follow-up office visit or hospitalization

When a patient follows up post hospitalization for AMI with his cardiologist what ICD-9 code does the cardiologist bill?

If it is within the 8 weeks (ICD-9)is it 410.72-Subendocardial infarction, subsequent episode of care?

Hospital readmission example:
Patient is admitted and treated with PTCA and DES for NSTEMI and then is discharged PDx 410.71 plus procedure codes 00.66 PTCA, 36.07 Insertion DES, 00.45 Insertion one vascular stent and 00.45 Procedure on single vessel.

Patient comes back 3 weeks later with chest pain and is admitted to the hospital. It is determined to be unstable angina and they put in another stent in another vessel.

Do you assign:
410.72 Subendocardial infarction, subsequent episode of care plus 411.1 the unstable angina and
414.01 the CAD of native vessel
plus the procedure codes?
