Co-signature
This is sort of a spin-off of my question a couple weeks ago but I am still confused. Sorry.
What constitutes MD documentation sufficient for coding? If a HC provider (nurse, dietician, Physical therapist, etc) authors a document and sends it to an MD for co-signature, is this sufficient for coding? What if the MD makes an addendum, states, "I agree with the above" and signs?
In the prior thread, it seemed like there were facilities that accepted this documentation if it was simply co-signed and then there were other people who said that this was not acceptable. Is this something you would need established in the bylaws for your facility or is there any overall guidance on this issue?
