HCC code list?

I have spent > hour searching the web for a current HCC list of (Hierarchical Condition Codes). Is this really that hard to find, or could someone help me with where to look?

    It's what CMS utilizes as part of risk-stratification in the outpatient setting. There is good reason when we educate providers about inpatient documentation - to also have awareness of what is beneficial in their office setting. Increases their interest level.
    I want to utilize some of that terminology when creating Dictation Tools (lists of Medicare compliant terms for frequent specialty-specific diagnosis)so that it helps them in both settings.
    Hopefully I am grasping this accurately as it is a new area I am venturing into!

    Not familiar with HCC list.  I agree it would be very helpful for physician education as you explained in your reason to find.

    The attached document includes the current list of HCCs published by CMS fo=
    r FY 2012 & 2013. These dx can be captured in both the inpatient and outpat=
    ient settings and were initially designed for Medicare Advantage (HMO) capi=
    tated payments. The HCC list is also used for other Medicare payment models=
    (bundled payments, ACO payments) and is now being utilized by other payors=
    . The methodology and use of the codes varies between programs and payors.

    I will have to read the journal. We use 3M CDIS at my organization and I just noticed that a reference tab was added for HCC's. There is a brief description but will definitely need more info.

    I believe the above link speaks to HCC info, what they are, how they are used, etc. I think Kaiser here in California uses HCCs to manage their capitated senior population in their CDI program rather than the usual Cc/MCC/DRGs for CDI. Hope those interested find this useful.

    CDI should focus on HCC capture in addition to CC/MCC capture that affect DRG payments. Accuracy of DRG assignment as well as capturing the additional risk adjustors on the HCC list will affect future bundled payment determinations.
