I have always used an EHR myself and find that it is a timesaver, but like Ann, there are sometimes issues where active problems have been copied and pasted and I do have to send inquiries to clarify and make sure that they are still active problems. One thing that comes up is those who have chronic conditions listed as acute problems. For example, the active problem list in the medical record has "acute on chronic diastolic heart failure" and I am having to get the physician to clarify that it is just "chronic diastolic heart failure" as they are only receiving their P.O. medications.
One thing I thing that is going to be beneficial is that we are going to start a "copy and paste" pilot program where we are going to be monitoring for these issues. Can't wait until this project is started.
